Duties and Responsibilities:

• Conduct medication reviews and optimise prescribing practices within GP practices.
• Work collaboratively with GPs and other healthcare professionals to improve patient care.
• Provide expert clinical advice on medication use and safety.
• Support chronic disease management and deliver patient education to enhance health outcomes.

Qualifications:

• Registered with the General Pharmaceutical Council (GPhC).
• Independent Prescriber qualification is highly desirable.
• Strong knowledge of clinical guidelines and prescribing best practices.
• Excellent communication and organisational skills.
