Clinical Pharmacist Job in Chester

We are seeking a dedicated Clinical Pharmacist to join a well-established practice in Chester. You will play a key role in optimising medication management and improving patient outcomes.

Location: Chester

Key Responsibilities:

• Conduct medication reviews and provide expert advice on prescribing.
• Work closely with GPs and other healthcare professionals to ensure safe and effective patient care.
• Support patients with long-term conditions by optimising their medication regimes.
• Conduct audits and implement strategies for improved prescribing practices.

Requirements:

• Registered with the General Pharmaceutical Council (GPhC).
• Experience in a primary care or clinical setting preferred.
• Strong knowledge of medicines optimisation and prescribing protocols.
• Excellent communication and problem-solving skills.
